Good Morning to one and all I am one of the patients of your prestigious Medical Center last Tuesday, January 27, who had undergone Magnetic Resonance Imaging (MRI). I wish to thank all the staff/employees who extended their unselfish and dedicated assistance while I was at the Center. Special mention to Dr. Hidegunda Santos, Medicard coordinator, the HMO staff (the two ladies from the billing section), Mr. Oliver Hufana and his staff (MRI section), and the nursing students who assisted at the MRI section.

patients” – Last March I came here for check-up. I was then referred to Dr.
Espinosa so I went to his clinic. He sent me back here for EEG. The staff
downstairs at the front office did the EEG for me right away so I had the time
to go back to Dr. Espinosa who interpreted the result. It was done so fast that
we had the time to go back to Ilocos that same day. I thank the staff very much.
To Lorma as a whole, thank you so much and may God bless you all. All the staff
and employees of Lorma Hospital are very friendly. I’m very glad to see their
smiling faces every morning.

FRINGE BENEFITS
An Employee shall, after 6 months of continuous, faithful and satisfactory service, is entitled to actual sick leave with full pay for the period not to exceed fifteen (15) days per year. This benefit may, however, be given only if a medical doctor in the hospital requires the employee's hospitalization. This benefit is not cumulative. In order to sick leave, the employees must present himself at the OPD or consult a doctor and inform the personnel officer of his hospitalization at the first day of illness. Any absence prior to the medical examination will be considered unexcused and not paid. Absence due to treatments by other than Lorma doctor must also have the approval of a member of the Lorma active staff on the first day of claimed leaves. All illnesses occurring outside the province of La Union must have a note from the attending physician and the merit of this will be judge by the medical director or the assistant director in his absence.
a.) AFTER ONE YEAR An employee shall, after one year of continuous, faithful and satisfactory service, is entitled to seven (7) days vacation leave with full pay. b.) AFTER 5 YEARS An employee shall be granted fifteen (15) days vacation leave with full pay after 5 yeas of continuous service. c.) FOR EXCEPTIONALLY MERITORIOUS CASES An employee may be awarded additional vacation leave with full or partial pay at the discretion of the director and administrator upon favorable recommendation of the division chief concerned. d.) PAID BIRTHDAY LEAVE In line with Lorma's objective to further promote initiative and achievement, additional one day paid leave on employee's birthday is provided. This benefit is extended only to those who have been continuously employed in Lorma for 10 years and above. This birthday leave should be taken on the birth date or incase the said date falls on a non working day then the leave will be enjoyed on the earliest possible working day.
SCHEDULE OF VACATION LEAVE Schedule of vacation leave shall be arranged by department heads with the initial approval of the personnel officer and final approval of the division concerned. APPLICATION FOR VACATION LEAVE Request for vacation leave should be submitted to the department head and personnel officer for their approval at least one (1) week before its effectivity. Applications received later are automatically disapproved and leaves taken will not be paid. VACATION LEAVE ARE NON-COMMUTABLE i.e. the money equivalent thereof may not be claimed by the employee; also non cumulative, i.e. vacation leave not availed of seasonably shall be deemed waived or forfeited, without prior written consent by the department head and administrator.
An Employee be granted at least five (5) emergency leave with pay within a year. Emergency leaves are scrutinized and the merit of this are judge properly. Additional leave with pay may be granted at the discretion of the administrator and director upon favorable recommendation of the department head or division chief concerned.
An Employee may be granted a maximum cumulative 15 days leave of absence without pay within a year upon favorable recommendation of the department head and division chief. And days taken beyond fifteen days must be granted on a case to case basis and this requires prior written approval by the administrator and the director. DEFINITIONS: Emergency Leave - is limited only to emergencies like death, or terminal illness of immediate member of the family, and calamity resulting to destruction or damages, which require immediate action. Leave of Absence - may be considered as other situations requiring immediate attention.
