Good Morning to one and all I am one of the patients of your prestigious Medical Center last Tuesday, January 27, who had undergone Magnetic Resonance Imaging (MRI). I wish to thank all the staff/employees who extended their unselfish and dedicated assistance while I was at the Center. Special mention to Dr. Hidegunda Santos, Medicard coordinator, the HMO staff (the two ladies from the billing section), Mr. Oliver Hufana and his staff (MRI section), and the nursing students who assisted at the MRI section.

“A smiling face is the mirror of our body and can heal sick
patients” – Last March I came here for check-up. I was then referred to Dr.
Espinosa so I went to his clinic. He sent me back here for EEG. The staff
downstairs at the front office did the EEG for me right away so I had the time
to go back to Dr. Espinosa who interpreted the result. It was done so fast that
we had the time to go back to Ilocos that same day. I thank the staff very much.
To Lorma as a whole, thank you so much and may God bless you all. All the staff
and employees of Lorma Hospital are very friendly. I’m very glad to see their
smiling faces every morning.

Thank you for my pleasant stay at Lorma. Keep up the excellent
care & service. Dr. Nelson Gundran is very patient, good physician. He listens
well to patient’s complaints. He shows happy disposition. Hope he’ll be a fellow

everyday. I had been to big and modern hospital in Manila, and it’s only here
that we are given newspaper. Nursing personnel – prompt in doing things

FOR YOUR INFORMATION UPON ENTERING THE HOSPITALThe admitting officer will register your name and other details before calling your doctor, except in an emergency. If you were previously confined at Lorma Medical Center, you can help by giving the approximate date of your last confinement or by presenting your Golden Health Card. (If you do not have such a card please request it from the Billing Office upon discharge). If you are too ill to give the necessary information, a member of your family may register for you. The party responsible for your hospitalization will be asked to sign for you. Your consent for hospital care, for operation, or for other procedures will be taken. If you are unable to sign or if you are of minor age your parents or guardian will be asked to sign for you. The questions you will be asked regarding your family and social status are necessary for vital records. This; together with your subsequent chart, is carefully preserved in the Medical Records Section arid it may be of importance for future hospital care. I/We are told that God is a very great help in times of trouble. We can accept or refuse His help. We may drift with the winds of adversity or make use of His power. TIME OF ARRIVALThe hospital is open 24 hours a day, 365 days a year. For the early
completion of routine and otlier required laboratory procedures, it is advisable
to come during daytime -between 8 A.M. -6 P.M. for cases that are not emergency
in nature. WHAT TO BRINGOnly personal effects that you are sure to need such as comb, toothbrush and toothpaste, shaving equipment, slippers and underwear should be brought along. Although we provide pajamas and nightgowns, it is advisable to bring extras togetheiwith a dressing gown or bed jacket. We strongly advise against bringing watches and other valuables to the hospital. THE HOSPITAL STAFF AND MANAGEMENT ARE NOT LIABLE FOR ANY LOSS OF nIESE ITEMS. If you have cash or other valuables you should deposit these with the hospital cashier. GIVE YOUR PAYMENTS ONLY TO THE HOSPITAL CASInER AND DEMAND A RECEIPT . WARNING!
